Please note, to prevent spam, Google now requires reCaptcha after you hit the Submit button.  Please keeps on scrolling up if you don't see the reCapcha after submit.  You might need to do the reCapcha more than once.  Once Google is satisfied that you are not a robot, the form will be sent, and you should receive a message and also a copy of your completed form in your mailbox.  The board will be reviewing your proposal shortly.
